I know many here have a long list of "what if"s and one of them could be "what if I have an emergency", well - I had one and survived 🙂
I was shopping and fumbled so that I accidentally pulled out my nail (I noticed that I don't need to describe what that feels like). The pain acted as a vasovagal trigger and my blood pressure dropped enough that it resulted in a syncope (fainting). I was standing leaning over the cart not feeling that good, then I woke up on the floor with people around me including one staff on the phone. My things where spread out on the floor (hear band, earring, one breast form, glasses) and at that moment I was mostly glad that nothing broke.
I ended up in an ambulance and there they did of course ask me questions and do tests. When they asked for my name I said "kelly" but did fill in that my legal name is different, they just said "no problems, I of course have to register you with your legal name but I make a note that your name is Kelly".
I was fully dressed and I think that resulted in only two things that doesn't always happen, to get my EKG I lifted up my breast forms to place the electrodes and during the "what medication are you taking" questions they added "are you taking HRT?" (I'm not)
Other than that I had no problems, no bad looks or anything, only some comment like "nice boots".
Now this happened in Canada, Ontario, Ottawa to be exact - so I guess everyone won't have the same experience.
As for final outcome - the short version is that since my blood pressure and pulse always been on the low side I have less of a margin when something happens. When the body gets some vasovagal trigger (sudden pain, stress, standing for long periods, guess getting a nail ripped out goes here) the body reacts and may drop the blood pressure a bit more resulting in syncope (fainting). I did see in my journal that at one point it was 96/70 with heart rate of 77.
After resting for about 1.5h I suddenly woke up. In the span of 30-60 seconds I did go from "I'm about to pass out" to "I'm ready to get up and go home". The numbers where back up and all good but I did still stick around for more testing and so on and they turned out all good.

About 5 years ago I got into a car accident while en femme. They called an ambulance, but I didn't need to go to the hospital. I did give them my male name, no reaction.
About 2 months ago I was feeling ill for a couple of days. Driving, I couldn't really get it checked out. When I got home, I went to the doctor in male mode (my daughter had to drive) where I was immediately sent to the hospital and admitted. I was wearing very feminine panties at the time, no comment from the nurse who had to undress me. The night nurse was taking inventory of my personal belongings, and her voice trailed off a bit when she said "panties", but otherwise no comment.
Long story short, I had a large stone in my bile duct which caused sepsis. One result was an extremely low blood pressure (at times in the hospital, my diastolic or bottom number was in the 50's). I was weak, lethargic, difficulty speaking, but didn't faint because it didn't drop suddenly.
You might be able to get an artificial nail put on at a nail salon to protect the nailbed until the new nail grows in.

First of all, so glad you are OK. This has always been my (and my wife's) biggest fear. "What if something happens while you're dressed up?" she asks. I told her if I am outed during a life-saving procedure, so be it. Save my pretty ass but please don't cut the dress off of me and don't lose my shoes!
