I don't pass, not even close to it, so I don't go out fully dressed. However, I've been underdressing -- bra and panties primarily -- nearly my whole adult life (I'm now in my 70s). Except for doctor appointments.
But that all changed last year after a visit to my new endocrinologist. During a very thorough exam related to my gynecomastia, I admitted to him that I usually wore a bra but was afraid to wear it to doctor appointments so I didn't have it on that day (supposedly for the support of my much larger than normal breasts, but the crossdressers here know there's more to it than that). He immediately encouraged me to wear it to my appointments with him in the future -- and with any doctor. It's hard to describe how I felt after that visit -- relieved, excited, elated even -- the feelings were all very positive.
A few months ago I began having heart issues related to AFib and had to find a new cardiologist since I had just moved into the area where I live now. I decided I was going to underdress to all my doctor appointments in the future and be my true self.
The cardiologist would be a really good test. Almost every appointment with a cardiologist, at least in my experience, begins with an EKG, which means removing your shirt (or at least opening it) so the medical assistant can attach the sensors to your chest area.
At my first appointment with my new cardiologist, as expected I was asked to remove my shirt. I had worn a polo shirt and lifted it quickly over my head and removed it, fully exposing myself in a bra. Without any hesitation I started taking the bra off, assuming I needed to remove it, too. I had just reached around and unhooked it when the medical assistant said, "Oh, you don't need to remove that." Darn -- I was too nervous to try and hook it again so I just took it off and laid it on my shirt. Next time I would know better.
Over the last few months, I've had a number of appointments with the cardiologist. It's a fairly large office (at least ten cardiologists) and it seems I get a different medical assistant every visit. I always have to have an EKG done before the doctor sees me. Without fail, none of them have had any problem with me wearing a bra and keeping it on during the EKG. If I'm wearing a button-up shirt, I'm just asked to unbutton it; if it's a polo shirt, usually they just ask me to pull it up high enough to attach the sensors, including a couple of them above my bra. So my bra is on full display to them. Not one has said anything about the bra or even acted in any way like this was unusual.
I do wonder what they say to other staff members after I leave the office. But with HIPAA laws as they are in the US, I know they aren't supposed to say anything to anyone outside of the office, so I don't worry about it.
Have you had any similar experiences? Good or bad?

I had a Dr appointment a couple of days ago it was just to get a new script so I went underdressed. Just before I went in my Dr told me she has a Student Dr with her and asked if it’s ok that she sits in. I trust my Dr 100% so of course it was a yeah that’s fine. I’ve been suffering with a bit of depression over the last few years and when she asked how that was going I said I’ve started doing something that makes me happy again, crossdressing. She didn’t bat an eyelash just said “that’s great to hear” but I did hear a little gasp from the student. Then I had to take off my jumper to get my blood pressure taken which showed off my new rose gold charm bracelet and finally lift my shirt for some tummy measurements, lifting it just high enough to show a glimpse of my blue bra.
I’ve not been underdressed at the doctors (the only place I don’t underdress), but I did one time forget to take off my bra sufficiently early so when I was asked by the doc to remove my shirt, the tell tale signs of bra wearing were still obvious. The doc did not show any reaction at all, but simply got on with her examination. At the end of the consultation the usual: “is there anything else you would like to talk about?” seemed to be a bit more poignant than normal, but that may just be me being a bit paranoid.
Also, just very recently I sprained my ankle and had to go to the emergency room of my local hospital. The doctors and nurses did not bat an eyelid to my shaven legs at all. So after a few had examined me, I did not care anymore and had no sense of embarrassment and could concentrate on the pain.
I had a dermatology appointment a few months ago. As it was not sandal season yet my legs were shaved and my toes were painted. I was a bit nervous going to the appointment but needn't have worried. When I went into the exam room they had me take everything off but my underwear (male) and put on a gown. When the PA and nurse came in they examined me, which for dermatology is a full body exam. Nothing was said by either one during the exam. When the nurse came back in to tell me to get dressed she remarked on how pretty my toes were. It felt good to hear her say that.

Hello Trace, Like you I underdress all the time, but usually only knickers, not bra. A few years back had to go to the Doctors for some stomach trouble, he asked me to lay on the bed & lower my trousers a bit, so my knickers were on show. He pressed my stomach around a lot & clearly could see my knickers, but said nothing. So I am ok know to go to the Doctors underdressed, perhaps one day go fully dressed.

I’m a regular undressed and wear panties and a bralette daily ( pantyhose and a camisole in the cooler months ) and have worn these to doctors and specialists appointments. Now my female family doctor knows I’m a cd as my therapist suggested I let her know as I was dealing with some anxiety. But I do remember once when I had an appointment with a shoulder specialist and I was wearing everything I listed above as it was in the fall. When all of the sudden I thought to myself “ oh shit I might have to take my shirt off”. But to my surprise he didn’t ask and did his examination with my shirt on.
I see my doctor fully dressed without issue. They really don’t give a crap what we look like because they like everyone else, are too busy to care. I’m sure there are office comments but I suspect the talk would be about how much time and effort it probably takes me to get ready, and the fact those days are over for most women that work. When was the last time you saw a woman wearing hose? Or a skirt? Today it’s Walmart leggings and sandals for most of them.
Suzanne, both my dermatologist and my urologist have seen my panties. Neither even “seemed” to notice.
I no longer think about wearing panties. I have been discovered just about everywhere and nobody has ever said anything.
I’ve been a bit more selective about where I’ve worn a bra, however. I’ve never worn one to a doctor visit, but have been discovered, (again without a word,) when getting my hair cut or at the grocery store.

Only one of my recent doctors made any mention of my bra. I had an appointment with a new-to-me urologist last month, a doctor almost as old as me (70s). He did a VERY thorough physical as part of my first appointment with him, and when he had me lift my polo shirt, my bra was right in his face. He asked me why I was wearing a bra. I just answered that I have very large breasts (from gynecomastia) and he seemed satisfied with that, nothing more was said about the bra and I did not tell him that I was a CD. Other than that, the doctors and medical assistants I've seen over the last year have not even mentioned what I was wearing, other than that first medical assistant telling me I could have kept my bra on when getting that initial EKG.
BTW, no one has seen me in panties because I can't wear them anymore; I have urge incontinence issues and need to wear pull-up diapers. I'm trying to get that fixed so I can wear panties again; I really miss them. They've all seen my diapers, but they don't mention that, either, as a rule. Of course, they can see from my medical history that I have an incontinence problem, so there's no surprise there. It's a bummer getting old.
I have had quite a few ECGs, lately, echo tests, too. I never wore a bra to those appointments and don't plan on starting because I saw where they placed the electrodes.
What I did do, though, was eliminate my chest hair. Some electrodes wouldn't stay stuck. One nurse dry shaved my chest where she needed some. I helped myself and them by getting rid of it all.
Harriette, I understand! If you're getting an EKG, they will definitely see your bra if you're wearing one!
I've had at least 7 EKGs over the last two months. Except for the first medical assistant telling me I didn't need to remove my bra, it has not even been mentioned by anyone else, and I've had a different MA at every EKG (it's a big cardiology office). They've conducted themselves totally professionally and just did what they needed to do. No drama whatsoever. So I'm no longer even anxious about wearing a bra to my doctor appointments.
Wearing a bra every day and everywhere I go (for the last 30+ years, except for doctor appointments until this past year) has become such an integral part of who I am, I am very UNcomfortable when I don't have one on.
I never have with my GP, sometimes in the dentist’s chair. This summer I was in an “I don’t care who knows” mood so I wore black panties under some lovely white shorts to a surgical consult in Spokane. The blouse I wore had quite a plunge so I skipped wearing a bra. Well, for the exam the blouse came off. It had covered the panties when tucked but once out they could be clearly seen through the shorts. I had a sense that the surgeon clocked them but she didn’t miss a beat, just went on with the exam.
I have been to my dermatologist many times while underdressed. When she does the full body check I am given a hospital gown and asked to remove everything except for my underwear. Of course, when she moves to behind me to check my back and the back of my legs, she can obviously see my panties. She has never commented and just completes my check up as we continue to chit chat as normal. I am sure most people in the medical profession have seen everything before, so it does not bother me if they know I prefer to wear women's panties.

I am out as Cassie almost all the time. 4 years of pushing my fem side more and more. exactly 1 year ago I had my first appointment with a new (for me) doctor about my worstening arthritis. The first appointment I was only underdressed with panties. At the second appointment I went as fully Cassie. EVERYONE as SO nice. The first receptionist asked me by what name I wanted to go by and for my pronouns. One of the nurses told me about her nephew who was trans. The doctor ordered an EKG, so I had to take off my top, bra and forms. I have gotten past being embarrassed about how I'm dressed. A couple days ago I made a dentist appointment dressed as Cassie. They also asked for my female name and pronouns.
